  • Because TrueScience Scalp Serum contains soybean oil, anyone with a soy allergy should avoid using it. However, the soybean oil used in the scalp serum is protein free. If you still want to use it, test product on the inner forearm before using it on your scalp. Apply a small amount to your inner forearm. If no reaction occurs after a few minutes, apply to scalp as directed.

  • To change the name on your account to a business name, please fill out the Name Change Request Form, the Amended Distributor Form, and the Business Entity Form. These forms can be found in your eVO under the Communications tab, Library. Business documents are also required to verify ownership. Articles of Incorporation, Articles of Organization, Annual Report, etc… are all acceptable forms of business documentation. Business letterhead, cards, or marketing materials are not accepted as business documents. Forward all completed forms and business documentation together to compliance@lifevantage.com.

    To change the name of your account from a business name to your personal name, please fill out the same forms mentioned above and include formal documents to verify your legal name. Court documents, tax documents, or a passport are acceptable forms of a formal document. A Drivers License is not accepted as formal documentation. Forward all completed forms and formal documentation together to compliance@lifevantage.com.

    To change the name of your account from a business name to another business name, please fill out the same forms mentioned above. In addition, fill out a Business Entity Form for BOTH businesses.   • This would be considered a Transfer of Distributorship.

    To transfer an account, the account must be in good standing. The transferee/buyer cannot already have an existing customer or Distributor Account (even under a business name.) If the transferee/buyer has a spouse that has an existing account, they must have the same enroller. As such, if the transferor/seller of the account being transferred is under a different enroller, they will have to submit both an Enroller Change Form and Placement Change Form before they can transfer their account to that individual.

    To make this change, please fill out the Transfer of Distributorship form that can be found in eVO under Forms by going to the Communications tab, Library. Page 1 is filled out by the individual who is transferring the account. Page 2 is filled out by the buyer of the account. The buyer then needs to fill out a Distributor Application which can also be found in eVO under Forms.

    If either individual (seller/transferor and buyer/transferee) is a business, a Business Entity Form must also be submitted.

    After all documents are completed, please forward to compliance@lifevantage.com.
